The Family Medicine APP will provide comprehensive care for patients of all ages, focusing on preventive care, chronic disease management, and acute care services. The APP will coordinate care with specialists as needed. Primarily adult and geriatric patients, with some pediatric cases. Average 18-22 patients per day.
