This is an independent role, and candidates must have experience treating complex geriatric, internal medicine, and mental health patients.   • Schedule is M-F 8 am to 5 pm with on-call coverage

  • Provide primary care for PACE participants including preventive, acute, and chronic disease management
  • Collaborate with interdisciplinary team for care planning and oversight
